Hours of Service — Previous 7 Days
Required by FMCSR §395.8(j)(2). List on-duty hours you worked for ANY employer in the last 7 days.
Why this matters: Federal HOS rules track your driving/on-duty hours across all employers, not just Pro Logistics. We need to know your starting hours so we don't accidentally put you over the 70-hour limit on day 1.

FMCSA Clearinghouse Consent
Federal law (49 CFR §382.701) requires us to query the FMCSA Drug & Alcohol Clearinghouse before you can drive for us, and once a year after that.
What is the Clearinghouse? A federal database that tracks drug and alcohol program violations by CDL drivers. We can't see anything in it without your written consent.
Full Query Consent
I, the undersigned, hereby provide consent to Pro Logistics Services Inc. to conduct a full query (Pre-Employment and Annual) of the FMCSA Commercial Driver's License Drug and Alcohol Clearinghouse to determine whether drug or alcohol violation information about me exists.
What happens if you refuse? If you refuse to provide consent, FMCSA must prohibit you from performing safety-sensitive functions, including driving a commercial motor vehicle. So in plain English: no consent = can't drive.
Limited Query — Annual
I also consent to limited queries conducted annually to determine whether drug or alcohol violation information exists about me. I understand if a limited query returns information, Pro Logistics Services Inc. is required to conduct a full query within 24 hours, which will require additional consent.
How to register: You'll need a Clearinghouse account at clearinghouse.fmcsa.dot.gov. If you don't have one yet, we'll send you a link after you submit this application.

Road Test
Required by FMCSR §391.31. Every driver must pass a road test before driving a commercial motor vehicle for us — OR provide a valid CDL in lieu of the road test (per §391.33).
Road Test Details
Note: FMCSR §391.33 allows an employer to accept a copy of a valid CDL as equivalent to a road test for vehicles other than triples or hazmat tankers. PLS uses this exception for most drivers.
